These forms are designed to offer you professional, consistent, clearly outlined digital templates for your school-based music therapy assessments that you can fill-in, save and submit in a variety of formats – via email, upload to your districts online system or in print, as needed! There is a thorough form for more detail, and a short form that is more concise, depending on the needs/requirements of your schools. Also included are 2 sample assessments for each of the form types to give examples of the completed forms. The suggestions in the sample assessments as well as the fillable areas on the forms serve to provide considerations as you go through the assessment process and important information to help you as you fill out the forms. These forms should be used in conjunction with and NOT as a substitute for a validated assessment process, such as the Special Education Music Therapy Assessment Process (SEMTAP).
